The Medically
Fragile Program is designed to serve adults with
disabilities and who may have experienced decreased abilities
due to aging and who have extensive and complicated healthcare
issues. An MR diagnosis is required for this program. The
goal of this program is to maintain and enhance an
individual's physical, cognitive, social and emotional
abilities that will result
in an optimal quality of life for each individual.
The Medically Fragile Program's professional
staff consists of a Licensed Practical Nurse,
Certified Nursing Assistants and a Driver/Aide. These
staff members consult with families, physical therapists,
occupational therapists, special education teachers, doctors,
case managers and residential providers to ensure a comprehensive,
individualized program to meet each individual's goals.
The Medically Fragile Program opened in 1997
with start- up money from the Loudoun County United Way.
In October 2000, the program moved in to their new facility
located at 71 Lawson Road in Leesburg. The program
continues to grow and will be expanding to a larger space.
