Medically Fragile Day Support

ECHOworks’ Medically Fragile Program serves adults with significant disabilities who have experienced decreased abilities due to aging or who have extensive and complicated healthcare needs. The goal of this program is to maintain and enhance an individual's physical, cognitive, social, and emotional abilities that will result in an optimal quality of life for each individual.

Our newly remodeled facility serves up to 20 men and women.  An intellectual or developmental disability diagnosis is required for this program.

The Medically Fragile Program's professional staff consists of:

  • A Licensed Practical Nurse
  • Certified Nursing Assistants
  • A Driver/Aide

Our staff consults with families, physical therapists, occupational therapists, special education teachers, doctors, case managers and residential providers to ensure a comprehensive, personalized program to meet each individual's goals during the day.

A typical daily routine includes social interaction, crafts and other activities, exercise time, plus a morning snack and lunch. Each person has a customized exercise program that works to enhance his or her strength, endurance, and range of motion. The daylong agenda allows the staff to oversee the healthcare needs of those in the program.
